2. Requests for Disclosure of Personal Information
For retained personal data, we accept requests for notification of purpose of use, disclosure, correction, addition or deletion, suspension of use, erasure, or cessation of provision to third parties (collectively, “Disclosure Requests”).

Download the “Personal Information Disclosure Request Form” from the link provided. Complete all required fields and send it along with identification documents by mail. Please clearly indicate your preferred disclosure method. If not specified or if your preferred method is not feasible, we will send documents via traceable courier service. After receiving your request, we may verify your identity via email or phone.

Note: Requests for records of third-party provision or receipt of personal information will be handled similarly. However, if survey responses provided to clients without identifying information cannot be linked to specific periods or topics, we may not disclose such records due to potential disruption to our operations.

If a representative submits a disclosure request, please mail the following additional documents:
① Legal Representative of a Minor or Adult Ward
Proof of identity: Copy of official ID (driver’s license, passport, etc.)
Proof of authority: Family register or resident certificate showing relationship between the individual and the representative
Certificate of registration for adult guardianship, if applicable
② Authorized Representative
Proof of identity: Copy of official ID (driver’s license, passport, etc.)
Authorization: Letter of attorney
(7) Fees:
No fee is charged for notification of purpose of use or disclosure.
(8) Cases Where Disclosure Requests May Be Denied:
① When identity cannot be verified due to discrepancies in addresses
② When proxy authority cannot be verified
③ When required documents are incomplete
④ When the data does not qualify as retained personal data
⑤ When disclosure may harm the life, body, or property of the individual or a third party
⑥ When disclosure may encourage or induce illegal or wrongful acts
⑦ When disclosure may harm national security, damage trust with other countries or international organizations, or be disadvantageous in negotiations
⑧ When disclosure may severely interfere with our operations
⑨ When disclosure violates laws or regulations
⑩ When cooperation with government or local authorities would be hindered by disclosing the purpose of use
3. Security Measures for All Personal Data
We implement appropriate safety measures based on the purpose of use, including:

(2) Organizational Measures: Appointment of responsible persons, defined roles, communication and reporting systems

(5) Technical Measures: Minimization of access privileges, prevention of unauthorized access
